Claim Application (Others) Form
Under FLOWNet > Request Forms > Claims there is an option called Claims Application (Others) Form (refer to figure below). This option is similar to Medical Claim form explained earlier, and the difference is to allow employees to submit miscellaneous claims such as phone bill, mileage, meal and etc (Except for medical claim).
You may click on the Next button to apply a new other claim application, or select the draft request forms according to its AgentID from “Request No” drop down list followed by the Next button (refer to Employee_applying_application_request for explanation). You will see the page below if you select either one of the options mentioned above (refer to figure below). From this page, you may see a similar form as Medical Claim form.
If you notice, there will be some difference of input fields for Claims Application (Others) Form compared to Claims Application (Medical) Form. There are 4 different input fields within this form:
Customer Name – Name of customers which involved within the claim. (example: serving client dinner)
From Place – Specify the departure point for transport or mileage claim.
Place To – Specify the destination for transport or mileage claim.
Mileage – Specify the number of kilometers travelled for mileage claim.
The History hyperlink also will be available within Claim Application (Other) Form (refer to figure below). It will show you the historical claim request with its status if you click on it.
Fill in all necessary information within Claim Application (Others) Form and select the “Complete” status. Click on the Submit button to escalate the request to respective superior.
The Claim Application (Others) Form submission process flow is mentioned in unifiedESS process flow and Employee_applying_application_request. Request submission is subject to superior's approval.
